Please note: registration does not constitute enrollment. Enrollment is granted on a first come/first serve basis. All dogs must be friendly around people and other dogs. Payment is due in full prior to the start of class to reserve a spot in the class. Fees are non-negotiable, non-refundable and non-transferable. All dog owners must provide documentation of current vaccination at the first class.

Please bring to class:
Vaccination records (Rabies and distemper) - must show current vaccination status
Flat collar or harness - no slip, choke chain, shock, or prong collars
4-6 foot leather, nylon or cotton leash -- no retractable leashes, please
Pea-size training treats in baggie or treat pouch (Wear clothing with pockets if bringing a baggie with treats.)
PAYMENT: Once registration is received, applicants will be notified of available space on a first come/first served basis. Upon notification of available space registrants may secure their space in class by remitting the class fee in one of two ways: 1) mailed check or 2) online payment. Payment must be received prior to the start date of the class in order to reserve a place in the class. Once payment is received registrants will be added to the class roster. Which payment method will you be using?
LIABILITY WAIVER: I understand that dog training is not without risk to myself, my dog, members of my family, or guests who might attend my training session. I hereby release CANINE CONNECTION LLC, its employees, volunteers, officers and agents from any and all liability of any nature, for any loss, damage or injury to any person, dog, any illness to dog, or anything that may have arisen out of or in any way connected with participation in training by CANINE CONNECTION LLC. Furthermore, I assume all responsibility for any loss, damage or injury to myself, my animal(s) or my property which may be caused by neglect or any act by any person connected in any way with the above said training. I hereby certify that my animal is up-to-date on Rabies and distemper vaccinations as required by Iowa state law and that my animal has no known history of fear or aggression toward other people or dogs. I understand that registration does not constitute registration and that once payment is received I am considered enrolled in the class (es) for which I registered and all fees are non-negotiable, non-refundable and non-transferable.
